ASCENSION SOUTHEAST MICHIGAN COMMUNITY HEALTH PARTICIPANT RELEASE AND WAIVER (Recreational Activities)
I (“Participant”) acknowledge that I desire to participate in the online exercise class(es) offered virtually by Ascension Southeast Michigan Community Health. Such participation is strictly voluntary. In connection with my participation, I waive any claims or cause of action against and release Ascension Southeast Michigan Community Health, its agents, servants, officers, employees, instructors, other participants, affiliated organizations, parents and sponsors from any liabilities for injury, illness, death, property loss or other damages which directly or indirectly may result from my participating in the above program. I represent to Ascension Southeast Michigan Community Health, as a condition of my participation, that I am in proper physical health and condition to participate in the program. I understand that this type of program may expose me to a greater risk of bodily injury that the risk I might have in other sporting or recreational activities, and I acknowledge that I am fully aware that such risk is an open and obvious danger. I also acknowledge that during the class(es) I will not be monitored by any trainer. As consideration for Ascension Southeast Michigan Community Health permitting me to participate in the above activities, I agree specifically to assume all risks of personal injury, illness, death, property loss or other damages associated with such activities, regardless of whether I engage in the activities in my home or elsewhere. This assumption of risk includes, without limitation, environmental, theft and contagion risks, in addition to the risks associated with the activities themselves.
Any person or engaging in such activities should consult with his/her physician before beginning such activities. Further, I understand that should I choose to continue similar activities outside of this online program, I assume all risks of personal injury, illness, death, property loss or other damages associated with such activities.
