Child must be present or mother in 3rd trimester at time of appt for convertible car seats (5lbs-65lbs). During the check, the caregiver must sign waiver and show proof of qualifying factor for need of car seat: WIC, Medicaid, PATH, MI Child, Head Start, Great Start, Bridge Card, Referral from social or case worker, Refugee Assistance Program, Social Security/ Disability (see attachment in above heading)
